App to Manage Chronic Medical Conditions
Take-home support that activates patients with chronic conditions can make a positive difference to their health, and positively impact bed space in hospitals* as well as clinician and other hospital staff time. Support is now available in the form of a mobile lifestyle app that can be tailored with information to suit specific medical disorders such as cancer, HIV, diabetes and bariatric conditions.
The lifestyle app has been developed by Imagineear Ltd in a creative and investment partnership with Chelsea and Westminster Hospital and its charity CW+. It gives patients the ability to monitor and control their condition and well-being in the privacy and convenience of their own smartphone.
Its development supports NHS policy aimed at transforming the patient experience and delivering improved outcomes through efficient and patient-centred care. Tim Kelsey, NHS England’s director for patients and information has said, “We must embrace modern technology to help us lead healthier lives, and if we want, to take more control when we are ill. Our ambition is to make the NHS a digital pioneer for our patients and citizens.”
The lifestyle app gives patients an active role in staying healthy, particularly when they are no longer being treated, or are between check-ups. The content, developed by medics, gives advice and provides incentives through goal-setting and rewards to contribute towards their own well-being and to avoid additional treatment or hospitalisation.
